MEDICAL REPORT
FAQ & Quick Reference Guide
You will need the following information to file a medical report:
– Horses Registered Name
– Breed, Sex, Age, Color, Weight & Temperature of the Horse 
– Name of Medication, Date/Time Administered, Amount, Strength & Route of Administration

Generally Allowed or Controlled & Banned Substances
Use the database Search Tools to easily find over 1200 equine medications and their industry standard classifications and allowed use. Allowed Medications if used as directed generally do not require a Medical Form, however there are some exceptions. Controlled Medications require a Medical Form if used within 7 days of competition and most require >24 hour withdraw times, however there are many exceptions. NSAID stacking is not permitted. Contact your veterinarian with any questions.

BANNED MEDICATIONS AND SUBSTANCES – Are not permitted at any time. ESC does not condone use of Banned substances. If withdraw times are not available for a banned substance, it is the responsibility of the agent or prescribing vet to obtain that information.
