Non-Selective NSAIDs Bute & Banamine If Combined Pose Health Risks

PALMETTO, FLA — For years, horse owners have heard the strict warning: never combine NSAIDs. Yet, the standard medical explanations don’t always click. To understand the severe health complications of mixing Bute (Phenylbutazone) and Banamine (Flunixin), it helps to look at how these drugs actually function—and how they don’t.

The Misconception of Performance Enhancement

  • No Superpowers: As humans, many of us have mixed ibuprofen, aspirin, and Aleve just to get through a rough day. The result? We feel closer to baseline, but it doesn’t grant us superhuman physical talents or push our performance beyond normal limits.
  • Limitations of Pain Relief: If you have broken bones or severe muscle tears, a handful of NSAIDs won’t magically make you walk. They dull the ache; they do not cure the underlying injury.
  • The Rule Changes: Recognizing this, several equine sanctioning organizations recently reclassified single NSAID use as a lower-tier therapeutic violation. They acknowledge that, when used correctly, these drugs do not artificially enhance performance or pose an immediate health risk.
  • Performance Enhancing: Neither Bute or Banamine can be combined with Equioxx (firocoxib) due to a performance enhancing effect.

The “Third Cup of Coffee” Danger

  • The Normal Routine: Consider a daily coffee habit. One cup changes your willingness to move forward. Two cups might have you outpacing everyone in the room.
  • The Tipping Point: It is easy to think a third cup will push you over the hump. Instead, that extra dose tears up your stomach, disrupts your gut, and ruins your productivity.
  • The Equine Reality: Combining Bute and Banamine is the equine equivalent of that toxic third cup of coffee—but with much more devastating, long-term consequences for your horse.

Why NSAID Combinations Fails the Horse

Both Bute and Banamine are non-selective NSAIDs. They chemically function identical paths inside the horse by indiscriminately blocking both COX-1 and COX-2 proteins.

When you give these two non-selective drugs together, they don’t provide double the pain relief. Instead, they trigger immediate protein shedding, destroy the stomach’s protective lining, and dangerously restrict blood flow to the kidneys.

There are other Non-selective NSAIDS that your vet will know, however I am listing here for general information – these should never be combined:

Other Non-Selective Equine NSAIDs

  • Ketoprofen: Frequently used for musculoskeletal pain and acute lameness. It blocks both COX enzymes and has mild anti-fever properties. [1, 2, 3, 4]
  • Carprofen: Used occasionally for joint inflammation. It is less common in horses than dogs, but it acts non-selectively at higher equine clinical doses. [1, 2, 3, 4]
  • Aspirin (Acetylsalicylic acid): Rarely used for acute pain today but sometimes prescribed for specific blood-clotting or eye conditions. [1, 2, 3, 4, 5]
  • Meclofenamic Acid (Arquel): A non-selective option for chronic joint and skeletal issues. It has a slow onset of action. [1, 2, 3, 4]
  • Naproxen: Historically utilized for equine muscle soreness and tying-up syndromes. It is less common in modern practices. [1, 2, 3]

Important Clinical Context

Meloxicam: This drug sits in a gray area. It is a “preferential” COX-2 inhibitor, but its safety profile drops and it acts non-selectively at high doses. [1, 2]

The Golden Rule

Always secure your veterinarian’s approval before administering any controlled medication. Avoid combining drugs of the same class, as the toxic side effects will multiply exponentially.
